We invite all caregivers of CCM patients to take this survey to help us learn more about the barriers they encounter in your caregiving role. We hope to learn more about your experiences and how our programs may evolve to help overcome these barriers. The survey takes about 20 minutes to complete and asks about your financial situation, household composition, support systems, access to care, and your caregiver burden.
